My 13″ sheltie is a Service Dog, so I realize that there would be some differences – your dog would most likely be allowed to travel in the cabin, but in a carrier.
We traveled from Houston, TX to Wroclaw, Poland last spring. I changed planes twice which was convenient – I could take my dog outside when changing planes in Chicago O’Hare (there are dog potty areas outside of the terminals). The trip was surprisingly uneventful, my dog slept most of the time and the flight attendants were very accommodating (we flew Lufthansa). There was some paperwork to prepare beforehand (ISO compliant microchip, vaccination paperwork approved by a state veterinarian etc), but no problems when arriving to the EU (they scanned my dog and compared that with the paperwork).
So altogether it was easier than expected, but then my dog is used to traveling and can stay quiet for long periods of time.
